Payment Terms & Conditions 1. Legal Entity This agreement is with Cornerstone Bariatrics LLC (doing business as Dr. Sandy Bariatrics), located at 3085 Beyer Blvd STE A-103, San Diego, CA 92154 US.2. Nature of Payment Payments made through this portal are custom deposits or installments applied toward your total surgery balance. All transactions are processed securely in USD. 3. Medical Clearance & Prorated Refunds Patient safety is our ultimate priority. All surgeries are strictly subject to final medical clearance. If Dr. Sandy Martinez and her medical team determine you are not a suitable candidate for surgery due to health reasons, your refund will be prorated based on unspent funds up to the day of diagnosis:If disqualified before arriving at the hospital: You will receive a refund minus any logistical, laboratory, or administrative fees already utilized.If disqualified after hospital admission: Your refund will be calculated by deducting all medical, hospital, and logistical expenses incurred up to that exact moment. 4. Cancellations, Rescheduling & Travel Delays Booking a surgery requires reserving operating rooms and medical staff.
