1. Eligibility: Entrants must be registered for and present during the live webinar event. To be eligible for the drawing, entrants must submit a complete Health Questionnaire and receive official medical clearance/approval from Dr. Sandy prior to the drawing. Entrants must be 18 years of age or older.
2. Discount Scope: The 50% discount applies exclusively to the base surgery price offered by Dr. Sandy Bariatrics. The discount does NOT apply to hospital or facility fees, anesthesia, lab work, post-operative medications, nutrition consultations, or any additional add-ons/services.
3. Booking & Deposit Requirements: To claim the prize, the winner must pay the full $350 deposit (which will be applied directly toward the final base surgery balance) and officially schedule their surgery date to take place no more than three (3) months (90 days) following the live webinar event date. Failure to pay the deposit or book within this timeframe results in complete forfeiture of the discount.
4. Medical Discretion & Transferability: Dr. Sandy Bariatrics maintains sole discretion regarding patient safety and surgical candidate selection. If a winner is deemed medically unsuitable for surgery, the prize is forfeited. The prize is strictly non-transferable, has no cash value, and cannot be combined with any other offers, promotions, or prior payment agreements.
